What is Pre-Charting?

Pre-chart is a dedicated tab in Doximity Scribe where you can prepare clinical context before a patient encounter. In the Pre-chart tab, you can dictate, type, or paste notes or upload files (such as prior visits, referral letters, or intake forms) and save them as named pre-charts. When you're ready to start a Scribe session, you can launch a session directly from the pre-chart or attach a saved pre-chart from the dropdown when starting a new session, either way, Scribe uses that context to generate a more complete note.

 

Note: Pre-charts expire after 30 days.


How do I create and save a pre-chart?

In Doximity Scribe, navigate to the Pre-chart tab and click “New pre-chart”.

3. Save the pre-chart with a title so you can find it later.

 

On mobile:

  1. Go to Scribe and open your Scribe notes.

  2. Tap on Pre-charts or Pre-chart toggle.

            a. On iOS: Tap the blue plus icon (upper right corner)

            b. On Android: Tap Pre-chart (bottom right)

  3. Dictate, type, or paste your clinical context.

  4. Save the pre-chart with a title so you can find it later.

Your pre-charts are stored in your library and can be reused across multiple sessions.


How do I attach a pre-chart before starting a session?

The steps to attach a pre-chart depend on whether you're using desktop or mobile.

On web:

1. Open the pre-chart and click Start this note in the upper right-hand corner. This opens a new session with that pre-chart already loaded. Start recording when you're ready.

  2. Start a new Scribe session directly and select a saved pre-chart from the dropdown menu before you begin recording.

On mobile:

In your pre-charts list, tap Start or Start visit to the right of the pre-chart title. This opens a new session with that pre-chart already loaded. Start recording when you're ready.

 


How does pre-chart content appear in my note?

Your pre-chart content is summarized and used to enrich the current note contextually. For example, if you upload a prior note, relevant conditions or context are pulled forward automatically when your session completes. You can also add a pre-chart to an already-generated note at any time to incorporate additional context.


Can I add a pre-chart to a note that’s already been generated?

Yes.

1. Open the generated note in Scribe and click “Add Pre-chart.”

2. Select a saved pre-chart or type context directly.

3. Scribe will merge any relevant information into your existing note.
